This form should be used when submitting your Doctoral Thesis committee. You should submit the form at least 2 weeks prior to advancing to candidacy, as it needs to go through several levels of approval before your advancement date. If there have been changes to your previously approved committee, please submit your reconstitution request using this form at least two weeks prior to your defense.
